- In accordance with the "Families' Rights and Responsibilities Act", Public Chapter No. 1061, in order for a student to receive health care services by a school nurse or other person acting on behalf of the school system or receive counseling services from a school counselor or other school personnel, a parent or guardian must provide written consent. Parents must submit a form for each child enrolled in Smith County Schools.
